Expression and significance of soluble B7-H3 in human brain trauma with fracture healing

[Abstract] objective to detect the expression of soluble B7-H3 in peripheral blood of patients with traumatic brain injury complicated with fracture, simple fracture and simple brain injury, and to study its effect on the differentiation of mesenchymal stem cells into osteoblasts. Methods ELISA was used to detect the expression of soluble B7-H3 and Cbfa1, a marker of fracture healing, in patients with traumatic brain injury complicated with fracture, simple fracture, simple brain injury and normal volunteers. Alizarin red and real-time PCR were used to detect the effect of different concentrations of recombinant B7-H3 protein on the differentiation of mesenchymal stem cells into osteoblasts. The expression of b7-h3-h7-h7 was higher than that of h3-h7-bmscs in traumatic brain injury. Conclusion the increase of soluble B7-H3 in peripheral blood can promote fracture healing.
[Key words] B7-H3; brain trauma complicated with fracture; fracture healing; mesenchymal stem cells fracture healing is a dynamic balance process regulated by systemic or local factors, including osteoblasts and osteoclasts [1], and its healing speed is affected by many factors. Clinically, it was found that callus overgrowth, heterotopic ossification rate increased and fracture healing accelerated after craniocerebral injury [2-4]. Although many literatures have discussed the mechanism of traumatic brain injury promoting fracture healing from different angles, little is known about the mechanism of promoting fracture healing after traumatic brain injury.
B7-H3 (cd276) is a new member of B7 family, which has membrane binding and soluble modes. As a costimulatory or costimulatory molecule, it can mediate the immune regulatory response of T cells and is related to the occurrence and development of tumors. For its non immune function, B7-H3 knockout mice are prone to fracture, and the osteogenic differentiation of B7-H3 deficient cranial cells is blocked, which indicates that B7-H3 participates in the differentiation process of osteoblasts. Stimulating B7-H3 on the membrane of human mesenchymal stem cells with specific antibody 4h7 can directly promote their differentiation into osteoblasts [5-6].
Soluble B7-H3 (sb7-h3) is a soluble form of membrane binding type, which can be released by monocytes, dendritic cells, activated T cells and mb7-h3 + cells [7]. Studies have found that in patients with sepsis or pneumonia in children, the expression of serum sb7-h3 is increased, and it is positively correlated with the release of inflammatory factors [8-9]. The relationship between sb7-h3 and fracture healing has not been reported. In addition, stem cells, which are derived from different sources of osteoblasts, have the advantages of rapid proliferation and differentiation of osteoblasts. In order to study the role of sb7-h3 in the healing of traumatic brain injury combined with fracture, we investigated the relationship between the expression of sb7-h3 and fracture healing, and the effect of adding recombinant B7-H3 protein on the differentiation of mesenchymal stem cells into osteoblasts.
1. Data and methods
1.1 clinical data a total of 56 patients (36 males and 20 females) admitted to the emergency department of Southwest Hospital of Chongqing from February 2014 to November 2016 were selected. They were divided into TBI group (n = 13), fracture group (n = 20), TBI + fracture group (n = 23) There were 11 cases of fibula fracture, 10 cases of humerus fracture, 8 cases of ulna and radius fracture. 10 cases of healthy volunteers were selected as the control group. According to GCS score, patients with brain trauma at admission were classified as mild (6 cases) with GCS > 12, moderate (9 cases) with 9 ≤ GCS ≤ 12 and 8 cases with severe (8 cases) with 3 ≤ GCS ≤ 8. All cases were excluded from the preoperative neuropathology or bone related diseases, rheumatoid arthritis, diabetes and diseases treated with steroids or diphosphonic acid. This study was approved by the ethics committee of the First Affiliated Hospital of the Army Military Medical University (Third Military Medical University) and the informed consent of patients was obtained.
Blood samples were collected at - 500 ℃ in the morning and stored in the refrigerator for 3 hours.
1.3 ELISA detection human B7-H3 ELISA kit was purchased from ebioscience Co., Ltd., and human Cbfa1 ELISA kit was purchased from Shanghai huding Biotechnology Co., Ltd. The experiment was carried out according to the manual.
